HMS has run since 2002 (smoke analysis since 2005) at NOAA/NESDIS' Satellite Analysis Branch. Trained analysts manually inspect GOES, MODIS, VIIRS and AVHRR imagery and hand-draw smoke plume extents, classified into three density categories — light, medium and heavy. DOH recorded 1,112 pertussis cases and 54 deaths — all children under 5 — from 1 Jan to 30 Mar 2024, roughly 34× the 32 cases in the same period of 2023. Metro Manila had the highest count (58 of the first 568 national cases). Quezon City declared an outbreak on 21 March 2024 (23–25 cases, 4–5 infant deaths aged 22–60 days); Iloilo City declared a state of calamity; Bacolod City issued a health alert. DOH named CALABARZON, NCR, Western Visayas, MIMAROPA and Central Visayas as the regions with the most LGUs showing increases. By 23 April 2024 the national count reached 1,566. Rubella is airborne/droplet-transmitted and has been eliminated in the U.S. since 2004 (MMR ~97% effective), so U.S. risk is import-driven. Philippine DOH tracks rubella jointly with measles as measles-rubella (MR), recording 2,552 MR cases by 11 May 2024 with the epidemic curve then falling ~8% fortnight-over-fortnight. Varicella is one of the few pathogens with documented true airborne (aerosol) spread, R0 ~10–12 — comparable to measles. The U.S. 2-dose program (1995 one-dose, 2007 two-dose) cut overall incidence by >97% by 2019, with hospitalizations down 94% and deaths down 97% among people under 50. Breakthrough varicella still occurs in vaccinated people. Surveillance is partial: as of 2020 only 39 states conducted case-based varicella surveillance, and only ~45% of Minnesota's 247 cases in 2024 (4.0 per 100,000, up from 2023 with 17 outbreaks) were lab-confirmed. Cases surged from 7,063 in 2023 to 35,435 in 2024 (10 deaths) — the highest annual count in over a decade, peaking in November 2024 — stayed elevated in 2025 (13+ deaths per PAHO), and are running below 2025 in 2026.
